Pharmacy Quality Scheme (PQS)
Outcomes from Pharmacy Quality Scheme (PQS) 2021/22 – Please see the documents below summarising the outcomes of the 2021/22 PQS delivered by pharmacies across West Yorkshire.
Pharmacy Quality Scheme 2023/24
Guidance relating to the PQS 2023/24 is now available via the Community Pharmacy West Yorkshire website here: https://cpe.org.uk/quality-and-regulations/pharmacy-quality-scheme/. We will continue to update this page in due course with information and advice.
From 1st June 2023, CPWY will be emailing out regular PQS bulletins on the different domains and criteria. As these bulletins are circulated, we will link to them below so pharmacy teams can refer back to them.
CPWY has created the full guide below to help contractors plan activities to meet the requirements of the PQS 2023/24. Whilst most actions required do not have specific times when the activities need to be carried out, contractors may find this tool useful for planning and prioritising workload. The individual monthly action plans have been included below for ease of use.
This simple Action Plan Template can be used for any PQS criteria.
PQS Action: Use the Data Collection Form below to record the number of patients your pharmacy identifies and actions as required by the Respiratory Domain.
Medicines Safety & Optimisation Domain – High Risk Medicines – Anticoagulant Safety Audit
PQS Action: Contractors must have implemented, into their day-to-day practice, the recommendations for community pharmacy from the first Anticoagulant Safety Audit from the PQS 2021/22 found in the Community Pharmacy Oral Anticoagulant Safety Audit 2021/22 report. Please see below a summary of the report recommendations.

Remember to record your start and finish dates, you will need them for your MYS application. Please see link below to download a copy of the Audit Questionnaire.
