PQS 2021/22: Introduction for Contractors

Healthy Living Support Domain: Tier 2 Weight Management Services 

Pharmacy Action Plans for the Healthy Living Domain (PQS 2021/22) should include details of how to refer people to the NHS Digital Weight Management Programme for those with hypertension and/or diabetes or available Local Authority funded Tier 2 Weight Management Services (where the individuals meet the criteria for referral). Please see below locally provided Tier 2 Weight Management Services for each CCG area:


Bradford - https://mylivingwell.co.uk/

The Living Well Weight Management Programmes provide practical advice, recipes and tools both online and at weekly meetings.


Calderdale - www.calderdale.gov.uk/betterliving

The service offers 12 weeks fully funded Slimming World vouchers or 1-2-1 support from a Better Living Trainer depending on the individual’s needs and preferences. Anybody living in Calderdale, over the age of 18 and with a BMI of 25+ can be referred, although the website currently says over age of 30 or 27+ with co-morbidities).


Kirklees - Referrals can be made via : www.kirkleeswellnessservice.co.uk and take approximately 5 minutes.

Accepting referrals for clients with a BMI up to 50kg/m2. People can access 1:1 support only if referred by their GP, all other referrals will be offered access to healthy weight groups which can be delivered virtually or face to face.


Leeds - https://oneyouleeds.co.uk/manage-your-weight/

One You Leeds offers support to a people with BMI over 25 via both face to face and remotely (ie telephone/ video calls).


Wakefield – Patients with a BMI greater than 40 should be encouraged to contact the Wakefield Council Referral Services for further specialist support. Customers can either self-refer or a member of the pharmacy team can make the referral for them. Note: The NHS Digital Weight Management programme offers online access to weight management services to those living with obesity who also have a diagnosis of either diabetes or hypertension. Patients must meet the following criteria:

  • Over the age of 18.
  • Has a BMI of 30+ (adjusted to ≥27.5 for people from Black, Asian and ethnic minority backgrounds).
  • Has a diagnosis of diabetes (type 1 or type 2), hypertension or both.
