Leeds Supervised Consumption

The current contracts for the Pharmacy Supervised Consumption Service in Leeds expire on 30th June 2018. Leeds City Council are therefore inviting pharmacies to enter into a new contract to deliver this service from 1st July 2018. The contract documentation, including Terms & Conditions for the Service and a refreshed Service Specification can be found below.  All pharmacies located in Leeds who are able to meet the requirements as laid out in the documents are eligible to enter into a contract to deliver the service. It is imperative that pharmacists read the contract documentation and are able to commit to delivering the service as laid out in the specification before signing the contract. 

For ease, the element of the contract which needs completing has been saved separately (Section A – The Particulars).  Please complete the sections which are asking for the details of your pharmacy (blue text), sign the last page and return to Leeds City Council (details below). Please make sure you provide the full name, address and contact details of the pharmacy as requested. 

IMPORTANT: Pharmacists that are part of a company or chain should check centrally what the preferred arrangements are for entering into a contract as the owner / parent organisation may wish to have one contract covering multiple outlets.   Contract documentation must only be signed by a person who has the appropriate authority to do so on behalf of the pharmacy organisation they represent. If you are responding on behalf of a number of pharmacies, please also complete and return the Pharmacy Details Form listing the full details and the contact person for each pharmacy. 


The completed and signed documents can be returned by the following means:

Please return the signed document as soon as possible and no later than 8th June 2018.  If you are currently delivering the service and no longer wish to do so, please let Leeds City Council know as a matter of urgency so that they can make alternative arrangements for existing service users.

Forward Leeds

Alcohol and Drug Services in Leeds are changing from 1st July 2015.  DISC, Barca Leeds, St Anne’s Community Services, St Martin’s Healthcare Service and Leeds and York Partnership Foundation Trust will work jointly under the banner of Foward Leeds to deliver a more accessible, integrated service to support adults and young people across the city.

Using PharmOutcomes to Record Supervisions

Both Calderdale and Bradford supervisions are recorded and claimed on PharmOutcomes.

Step 1

Register the client in the Supervised Consumption- Registration Service.

This only needs to be completed once for each client.  Subsequent supplies are made under the Supervised Consumption- Supervision Service which will look up the client details from this registration stage.


Step 2

At the end of each dispensing episode, i.e. last day of dispensing/completion of dispensing from a FP10 MDA, the days that the medication was supervised should be recorded on PharmOutcomes. As the recording is triggered at the completion of a script this will allow pharmacies to record the necessary data and then to safely file the script ready for submission to the PPD.


Next to each date you need to record the action taken.  It is important that the action taken is recorded as below:

Script was for less than 14 days.  Mark all days not covered by the script with this option.


Choosing the supervised option against a date will generate a claim.  Ensure that supervised is recorded next to a date only when the script indicated a supervision AND the client came to collect the medication AND the dose was supervised.


Supervisions should be recorded in a timely fashion onto PharmOutcomes (i.e. on the last day of dispensing).
