Leeds Needle Syringe Provision Service

Leeds Needle Syringe Provision Contract - Updated 2021

The Leeds Needle Syringe Provision Service is currently delivered by Forward Leeds from 3 separate locations:

  • Irford House, Seacroft Crescent, LS14 6PA
  • Armley Park Court, 9 Stanningley Road, LS12 2AE
  • 74 Kirkgate, LS2 7DJ

Forward Leeds is the alcohol and drug service in Leeds for adults, young people and families.  In conjunction with Forward Leeds, the service is also delivered by a number of city-wide pharmacies.  Pharmacies, with the assistance of the client, must fully complete a What Works prescription form.  Pads of the What Works forms are provided by Forward Leeds.  Please contact Forward Leeds if you run out.  Details from the What Works forms must then be input on to PharmOutcomes.  This not only works out what pharmacies need to be paid but also collates useful statistical information about the service.

Service Commissioner: Forward Leeds (Humankind), https://www.forwardleeds.co.uk/

Service Lead: Anne-Marie McMullan, 0113 887 2477

For any queries about the service, email the generic email box spocreferrals@nhs.net and mark for the attention of Anne-Marie McMullan.
