Medication Administration Record (MAR) Chart Scheme

This service is commissioned in Bradford and Leeds only.

Pharmacies who are commissioned to provide this service help support domiciliary care workers by preparing a medication administration record (MAR) sheet when a prescription is presented for a patient assessed by the local authority as requiring the service.

Bradford Medication Administration Record (MAR) Chart Scheme

The Bradford MAR Chart scheme is only for patients who are referred to the pharmacy by Bradford Adult Community Care.  Selected pharmacies in Bradford are contacted by Bradford Adult Community Care when they identify a service user who requires MAR Chart.


If you want to be added to the list of pharmacies that Bradford Adult Community Care use then you would need to contact them, they may then refer service users to you. This is a very small service and we would not really expect any more service users than we currently see.
