Connect with Pharmacy

Connect with Pharmacy uses PharmOutcomes to enable patients to be referred from hospital to their community pharmacy. This allows patients who have been identified in hospital as needing additional support with their medicines to be referred, (through a safe and secure digital platform), to their local community pharmacy when they are discharged.

The initiative has shown that patients who see their community pharmacist after they've been in hospital are less likely to be readmitted and, if they are, will have a shorter length of stay.

We have had a sucessfull rollout of this service from Leeds Teaching Hospitals and Calderdale and Huddersfield Foundation Trust.  In 2019, we are planning further rollout from Bradford Royal Infirmary. Leeds and York Partnerships (mental health and learning disabilities services) and Airedale General Hospital.
