Connect with Pharmacy

Connect with Pharmacy uses PharmOutcomes to enable patients to be referred from hospital to their community pharmacy. This allows patients who have been identified in hospital as needing additional support with their medicines to be referred, (through a safe and secure digital platform), to their local community pharmacy when they are discharged.



Connect with Pharmacy in Leeds

Leeds Teaching Hospital Trust (LTHT) have funded this service improvement project.  Community Pharmacy West Yorkshire (CPWY) are working in partnership with the Leeds Teaching Hospital Trust (LTHT) in the development and implementation of this project.

Connect with Pharmacy in Calderdale and Huddersfield

Phase 1 of Connect with Pharmacy goes live in Calderdale and Huddersfield on the 5th November 2018.

From this date hospital discharge letters from Calderdale Royal Hospital and Huddersfield Royal Infirmary will be sent electronically, via PharmOutcomes, to community pharmacies in the Calderdale and Huddersfield areas. This will replace the fax system currently used by the trust and initially will only be for patients who use a Monitored Dosage System.
