Connect with Pharmacy
Connect with Pharmacy uses PharmOutcomes to enable patients to be referred from hospital to their community pharmacy. This allows patients who have been identified in hospital as needing additional support with their medicines to be referred, (through a safe and secure digital platform), to their local community pharmacy when they are discharged.
The initiative has shown that patients who see their community pharmacist after they've been in hospital are less likely to be readmitted and, if they are, will have a shorter length of stay.
This service has now been successfully rolled out from the following hospitals:
- Airedale General Hospital
- Bradford Royal Infirmary
- Calderdale & Huddersfield
- Leeds Teaching Hospitals Trust
- Leeds & York Partnership Trust
- The Mid Yorkshire Hospitals NHS Trust
- South West Yorkshire Partnership NHS Foundation Trust

Evaluating Connect with Pharmacy
The results showed a reduction in re-admissions and potential post-intervention length of stay, indicating there may be further benefits for our older patients’ experiences and hospital flow.

Connect with Pharmacy in Airedale
Connect with Pharmacy went live in Airedale on the 9th September 2019. From this date hospital discharge letters from Airedale General Hospital will be sent electronically via PharmOutcomes, to community pharmacies.
Connect with Pharmacy in Bradford
Phase 1 of Connect with Pharmacy went live in Bradford on the 15th July 2019. From this date hospital discharge letters from Bradford Teaching Hospitals NHS Foundation Trust are sent electronically, via PharmOutcomes, to community pharmacies.

Connect with Pharmacy went live in Calderdale and Huddersfield on the 5th November 2018. We have since rolled this service out to North Kirklees and other surrounding areas.
