Leeds Blood Pressure Wise Service
- To increase the detection and management of hypertension and atrial fibrillation.
- To provide easy access to blood pressure monitoring through community pharmacies
- To develop and pilot a new method of data collection and data sharing using a SystmOne Community Module.
- To encourage self-management and healthy lifestyles to reduce risks of developing hypertension
The service will be offered to any person over the age of 35 years old that meets the criteria outlined in annex 3 of the service specification (below).
