CPWY Connect Event – February 2020 

Community Pharmacy West Yorkshire are holding our February 2020 CPWY Connect Event in Bradford.  Further events will be held monthly across the West Yorkshire area up to summer.  It is planned that this event will include the following topics:

  • Community Pharmacy Consultation Service (CPCS) – CPCS overview, question and answer session to discuss issues.

The Leeds Atrial Fibrillation (AF) Patient Pathway in Leeds – Integrating Community Pharmacy into the Pathway 

This event will complement the anticoagulation events held in January in Leeds and March 2020 in Bradford.  This Leeds AF Pathway event will focus on supporting patients with AF and will include practical information, outline how patients will be referred to community pharmacy for an anticoagulant NMS and provide an opportunity to meet with pharmacy teams from the CCG and GP practices. 

The number of patients prescribed anticoagulants is increasing and community pharmacy is in a great position to support these patients.  The event will:

 

  • Assist pharmacy teams in the conversations that they have with patients prescribed anticoagulants.
  • Improve your confidence and knowledge in delivering an anticoagulation NMS.
  • Introduce the anticoagulant NMS referral letter and how this will be used in practice.
  • Provide a brief update on anticoagulants and anticoagulation.
  • Give an overview of the Leeds AF project.
  • Outline the role of pharmacy from all sectors, including how we can best collaborate and integrate.
